Reporting Requirements

This form is provided for use by a physician, physician assistant, or nurse practitioner to report an individual whose driving ability may be affected due to some physical or mental health. 

In this webinar, attendees learned about how to engage older drivers in a thoughtful manner, how to do roadside screening, and where to refer older drivers for more resources to keep them, and our roadways safe.
